Special Events Application

  1. The Unified Business Identifier (UBI) number is a nine-digit number issued by the State of WA used to identify that a business is licensed in WA.

  2. Indicate set-up time, actual event time and tear down.

  3. Describe your event. What type of activities, is it family oriented or adult oriented, who does the event benefit..?

  4. Two Contact Persons:

  5. Does your organization have a planning committee?

  6. Will your event take place on Centennial Trail?

  7. Are you requesting streets to be closed?

  8. Will street barricades or safety fences be used?

  9. Will vendors be operating booths or tables?

  10. Will amplifiers or microphones be used?

  11. Will access to adjacent properties be affected?

  12. Will generators or other power be used?

  13. Will food vendors be involved?

  14. Will stages or platforms be constructed?

  15. Will cooking facilities use portable fuel tanks?

  16. Will there be adequate restroom facilities?

  18. Based on the street closures and traffic restrictions identified by the Sponsor, the City will prepare an appropriate traffic control plan. The Sponsor is responsible for setting up and monitoring the barricades and signage. Barricades must be monitored throughout event.

  19. Please upload a map of requested street closures or route for run/bike type events.

  20. Will barricades, barriers or safety fences be used? If yes, the City will provide a barricade plan and sample No Parking template that the Sponsor shall implement.

  21. Will you be renting barricades from the City? If yes, a contract with the City is required. Arrangements to rent barricades from the City shall be made at least seven days in advance of the event. If an alternative provider will be used, please provide the name of the purveyor.

  22. If you will be using generators or connecting to power have you applied to Washington State Labor & Industries for an electrical permit?*

  23. What methods do you propose for notifying adjacent properties or trail users? When will notification occur? Please describe below:

  24. Do you have insurance? INSURANCE REQUIREMENTS: A Certificate of Insurance and Policy Endorsement naming the City as Additional Insured in the minimum amount of $2,000,000 per occurence is required for all special events. *

    The certificate and the policy endorsement must be submitted an be acceptable to the City prior to issuing the special events permit.

  25. HOLD HARMLESS STIPULATION: Permittee covenants and agrees to indemnify, defend and hold harmless the City, its officers, agents and employees from and against any and all claims, actions, damages, liability, cost and expense, including reasonable attorney’s fees in connection with or occasioned, in whole or in part, by any act or omission of Permittee, its officers, agents, employees, customers, or licensees, or arising from or out of Permittee’s failure to comply with any provision of this Permit, regardless of whether it is alleged or proven that the acts or omissions of the City, its officers, agents or employees caused or contributed thereto. The parties mutually agreed upon this waiver. This indemnity provision shall not apply in the event any acts or omissions of the City were the sole cause of any such damage or injury. To the extent any of the damages referenced herein were caused by or resulted from the concurrent negligence of the City, its agents or employees, and the Permittee, its officers, agents, and employees, this obligation to indemnify, defend and hold harmless is valid and enforceable only to the extent of the negligence of the Permittee, its officers, agents, and employees. By submitting this application and entering my name below, I agree and acknowledge my electronic signature is valid and binding and contains the same force and effect as my handwritten signature and may not be denied legal effect solely because it is conducted, executed, or prepared in electronic form. I acknowledge that the information submitted is true and correct and that I have the authority and I am authorized to submit and execute this application. By submitting this application for a special event permit, I acknowledge that the application the special event permit is subject to suspension or revocation if false or misleading information is provided; and violation of any conditions and/or requirements of the Snohomish Municipal code or any permit conditions may result in the revocation or denial of an application or permit for a special event and the forfeiture of any fee paid.
